What is the Service Delegate Authority form used for?

  • The CommBiz Service Delegate Authority form is a form you generate online in CommBiz when:

    • You’re setting up your CommBiz service for the first time
    • You want to add Service Delegates to your CommBiz service
    • You want to manage the list of Service Delegates on your CommBiz service

Completing the Service Delegate Authority form

  • The Service Delegate Authority form has 3 sections:

  • Section 1 – Declaration and Consent

    This section confirms what Service Delegates can do within CommBiz.

  • Section 2 – Service Delegates

    This section confirms how many Service Delegates are needed to authorise a request.

    The table lists all new and existing users who will have Service Delegate permissions once we process this request.

  • Section 3 – Declaration and Acknowledgement

    This section includes the declaration and acknowledgement. This section must be signed by each Entity (Directors or Authorised Officers) with an account linked on the CommBiz service. By signing, they agree to:

    • The number of Service Delegates required to electronically authorise requests
    • Allowing Service Delegates to electronically authorise requests for their accounts on their behalf
    • The CommBiz Terms and Conditions

  • President; or 
  • Vice President; or
  • Other Authorising Officer

How to sign forms

  • We're unable to accept DocuSign, digital, electronic or cropped signatures so you'll need to sign in full with a pen. Your signature must match your identification documents (e.g. driver licence).
